BMI made coffee drinkers and non-drinkers look much the same. Their body composition did not. At the highest reported intake, similar weight for height concealed substantially less total and visceral fat.

Researchers analyzed one-time data from 2,264 46-year-olds in the Northern Finland Birth Cohort, combining self-reported coffee intake—nearly all of it filtered—with fasting metabolic tests, bioimpedance-based body-composition estimates, and hormone measurements. Compared with non-drinkers:

  • People reporting at least five cups daily averaged about 9% less fat mass and 6% less visceral fat, despite nearly identical BMI.
  • They also averaged 5% more skeletal muscle, but that association did not persist when men and women were analyzed separately; the high-intake group contained more men.
  • Higher coffee intake tracked with lower branched-chain amino acids in both sexes; men also showed lower insulin measures and greater estimated insulin sensitivity.

These contrasts came from a single snapshot rather than a coffee intervention. Only 70 participants drank no coffee, and heavier coffee drinkers were more often current smokers, so differences between the groups may partly explain the findings. Cup size and additions were undefined, and bioimpedance estimated body composition indirectly.

The testosterone pattern pulled in opposite directions. Among men, each additional daily cup accompanied about 8 nanograms per deciliter more total testosterone and 2 nanograms per deciliter more bioavailable testosterone—but also 0.57 nanomoles per liter more sex hormone-binding globulin (SHBG) and slightly lower calculated free-testosterone measures. Across a modeled four-cup difference, the SHBG association amounted to about 2.3 nanomoles per liter against an adult male reference interval of roughly 10 to 57. With hormones measured only once, this looks more like a subtle shift in testosterone distribution than a straightforward boost.

Set alongside FMF's broader evidence that long-term coffee use is associated with better metabolic health, even though caffeine can transiently reduce insulin sensitivity, the findings suggest that habitual coffee use may accompany a leaner metabolic profile without changing BMI. The clearest signal was not lower body weight, but a different partitioning of it—an association BMI missed and the testosterone measurements did little to simplify.
