Omega-3 plus low-dose aspirin improved outcomes in advanced gum disease over one year, with results similar to antibiotics. Digest

For some people with periodontitis (gum disease), removing plaque and hardened deposits below the gumline is not enough to bring the disease under control. A year-long clinical trial tested whether antibiotics, omega-3 plus low-dose aspirin, or both could improve treatment outcomes in advanced cases.

The study included 109 adults with advanced periodontitis who received deep cleaning and maintenance visits throughout the following year. They were randomly assigned to placebo, a 14-day antibiotic course, 3 g of omega-3 plus 100 mg of aspirin daily for six months, or both. The main comparison was the proportion in each group who ended the year with no more than four gum pockets measuring at least 5 millimeters deep. Deeper residual pockets signal less stable disease control and a greater risk of progression.

  • Omega-3 plus low-dose aspirin raised treatment success from 23% with placebo to 58%, while antibiotics reached 59%. The response emerged by 6 months and remained evident at one year.
  • Combining antibiotics with omega-3 plus aspirin offered no clear advantage.
  • Both treatments reduced the number of deep gum pockets more than placebo. By contrast, average plaque, bleeding, pocket depth, and other measures of gum health improved similarly across groups.
  • The four groups did not clearly differ in reported severe adverse events.

Antibiotics and omega-3 plus aspirin target different parts of the same disease process. One reduces bacteria that help drive tissue damage. The other may shift the body's response away from persistent inflammation and toward resolution and repair. Although the treatments act through different mechanisms, both may move the mouth toward the same stable state. Once either route approaches the clinical improvement possible in this setting, combining them may leave little room for an additional gain.
