Creatine helps regenerate ATP in muscle, brain, and other tissues. In this clip from Dr. Rhonda Patrick's appearance on The Jordan Harbinger Show, she discusses creatine monohydrate for physical performance and emerging cognitive applications. A daily maintenance dose of three to five grams is well established for supporting repeated high-intensity effort, strength, and lean-mass gains when combined with resistance training. [1]
Brain creatine response can vary with baseline diet, age, stress, sleep loss, dose, and outcome. In a small magnetic-resonance spectroscopy study, six adults who took 20 grams per day for four weeks had an average increase of about 8.7 percent in brain creatine. A recent meta-analysis also found small benefits in selected cognitive domains, including memory and processing speed, with responses that may depend on age, diet, baseline stores, stress, dose, and the cognitive test used. Higher-dose creatine has also been studied during severe sleep deprivation. In a randomized crossover experiment, 15 healthy young adults received a single dose of 0.35 grams per kilogram after being awake for about 21 hours. Creatine improved selected cognitive-task and brain-energy measures compared with placebo, providing an early signal that increased creatine availability may help the brain meet an acute energy challenge. Creatine monohydrate remains the best-studied form, and current finished-product testing can help confirm supplement identity and potency. [4]

Jordan Harbinger: Creatine has been used for training for years. Does it also have cognitive benefits?
Dr. Rhonda Patrick: Creatine helps regenerate ATP, the energy currency used by muscle and other tissues. Creatine monohydrate is the best-studied form. For training, three to five grams per day is the usual maintenance range. Loading is optional.
The largest gains in strength and lean mass occur when creatine is combined with resistance training. It supports repeated high-intensity effort and training volume. It does not directly replace protein or exercise.
Brain effects are less certain. A very small magnetic-resonance spectroscopy study gave six volunteers 20 grams per day for four weeks and found a modest average increase in brain creatine. It did not establish that 10 grams is a threshold or that muscle takes all of a five-gram dose before the brain receives any.
Controlled cognition studies are mixed. A recent meta-analysis found small benefits in selected domains, such as memory and processing speed, but no clear overall cognition or executive-function benefit. Results may depend on age, diet, baseline stores, stress, dose, and the test used.
One small crossover experiment studied 15 healthy young adults during about 21 hours awake. Participants received a single dose of 0.35 grams per kilogram, which is roughly 20 to 25 grams for many adults. Creatine improved selected cognitive tasks and brain-energy measures compared with placebo during severe sleep deprivation.
That experiment does not show that creatine restores sleep, removes every effect of sleep loss, or should be taken at 20 to 25 grams after an ordinary poor night. It also does not establish the long-term safety or benefit of routine high-dose use. Sleep remains necessary.
I take 10 grams per day and sometimes more during travel. I have noticed less afternoon sleepiness, but that is a personal observation and could be a placebo effect. It should not be treated as a universal protocol.
Jordan Harbinger: What form and product quality should people consider?
Dr. Rhonda Patrick: Creatine monohydrate has the strongest evidence, broad availability, and low cost. Some creatine gummies have failed independent potency tests, partly because creatine can degrade in moist, acidic products. That does not mean every gummy contains no creatine. Look for current finished-product testing rather than relying on the dosage claim alone.
Third-party certification can reduce uncertainty about identity, potency, contaminants, and banned substances. It does not prove that a supplement is effective for every goal.
Large doses can cause gastrointestinal discomfort and water-weight gain. Splitting a loading dose can improve tolerance. Creatine can also raise serum creatinine without reducing true kidney function, so users should tell clinicians before kidney testing. People with kidney disease, pregnancy, or relevant medicines should seek individualized guidance before high-dose use.
