Creatine monohydrate supports rapid ATP regeneration during repeated high-intensity effort. In this clip from Dr. Rhonda Patrick's appearance on Huberman Lab, she explains that an optional loading phase of about 20 grams per day for five to seven days can saturate muscle quickly. A routine dose of 3 to 5 grams per day reaches a similar level over approximately three to four weeks. When paired with resistance training, creatine can modestly improve strength and lean-mass gains. [1]
The brain also uses creatine, although its uptake and cognitive effects vary. A meta-analysis of 16 small trials found modest benefits for memory and selected timed outcomes, with less consistent effects across overall cognition and executive function. The most promising experimental setting may be severe sleep loss, when cellular energy demand is high. [2]
In one randomized crossover study of 15 healthy adults, a single dose of 0.35 grams per kilogram changed brain-energy measures and partially reduced deterioration on selected cognitive tasks during 21 hours without sleep. A later 29-person study using 0.2 grams per kilogram found its clearest benefit in reasoning, with mixed results for memory, accuracy, fatigue, and sleepiness. These studies help clarify creatine's role in brain energy under acute stress, while 3 to 5 grams remains the best-established routine dose for exercise benefits. [3] [4]

Dr. Andrew Huberman: I have used creatine for many years. Older protocols used a loading phase, 5 grams several times per day, followed by a 5-gram maintenance dose. Some people also cycled off and reloaded.
Dr. Rhonda Patrick: Loading is optional. About 20 grams per day, divided for five to seven days, saturates muscle quickly. Three to five grams per day reaches a similar level over about three to four weeks. Routine washouts, cycling, and grape juice are not required.
Creatine monohydrate raises muscle creatine and phosphocreatine. This supports rapid ATP regeneration during repeated high-intensity work. It is not an anabolic steroid. When paired with resistance training, it can modestly improve strength and lean-mass gains.
The brain also uses creatine, but brain uptake and cognitive effects are less certain. Small heterogeneous trials report benefits in selected memory, attention, or processing-speed measures. Other trials are null. Ten grams per day is not an established cognitive threshold, and muscle does not simply consume the first 5 grams before creatine can reach the brain.
The clearest experimental signal may occur during severe sleep loss. A later 29-person study using 0.2 grams per kilogram found benefits in some tasks but not in fatigue, sleepiness, or several other outcomes.
These studies did not show that creatine feels like eight hours of sleep, restores the full biology of sleep, or makes sleep-deprived driving safe. They also do not establish a routine 20-to-25-gram travel protocol.
An uncontrolled pilot gave 20 grams per day to 20 people with Alzheimer disease for eight weeks. Brain creatine rose, and some cognitive scores improved. Without a control group, the study cannot establish treatment efficacy or be generalized to mild cognitive impairment.
My own 10-gram daily use and occasional higher travel dose are personal experiments. I have said the perceived afternoon benefit may be placebo. Ordinary mental work, depression, traumatic brain injury, and total sleep deprivation are not interchangeable forms of “brain stress.”
Creatine is generally kidney-safe at studied doses in healthy adults. It can raise serum creatinine without reducing true filtration, which can complicate kidney tests. Evidence is strongest for routine 3-to-5-gram doses, not chronic 10-to-25-gram use. People with kidney disease, abnormal renal tests, pregnancy, or relevant medications should seek clinician guidance.
