Migraines are not just bad headaches. They are a complex neurological disorder that can disrupt the entire body during an attack, causing pain, nausea, sensitivity to light and sound, and even difficulty thinking clearly. While the full picture is still being uncovered, researchers now know that inside the brain a cascade of events unfolds: nerve pathways become overactive, inflammatory processes are triggered, and key neuropeptides such as calcitonin gene-related peptide (CGRP), substance P, and neurokinin A amplify pain signals and contribute to blood vessel dilation. Together, these changes heighten pain sensitivity and drive many of the symptoms people experience.[1] But here is the good news: with small, consistent lifestyle changes, you can shift your brain's "migraine threshold", making attacks less likely and less severe.
This simplified flowchart outlines how clinicians assess and confirm a migraine diagnosis. The process begins with a detailed medical history, including symptoms, risk factors, and family background. A physical examination and, when needed, medical tests help rule out other possible causes of headache. If migraine is suspected, clinicians determine whether it occurs with aura—involving visual or sensory disturbances—or without aura. The frequency of attacks is also considered: symptoms on more than 15 days per month indicate chronic migraine, while fewer days suggest episodic migraine. Together, these steps guide an accurate diagnosis and support individualized treatment planning.
